Staying healthy may seem like hard work at first, but by making a few changes to your daily habits and lifestyle you can make your health work for you! This sheet will give you easy steps you can take to prevent, delay or manage your type 2 diabetes, reduce health risks, and improve your health over the long term.

ASCVD Risk Estimator

Get an estimate of your risk for cardiovascular disease over the next 10 years and set a goal with your doctor to reduce that risk! Simply plug in your current lab values for total cholesterol, HDL cholesterol, and LDL cholesterol (ask your doctor for these values if you don’t have them.) Add your blood pressure (systolic is the “top” number; diastolic is the “bottom” number) Add your age, sex, race, history of diabetes (Y/N), smoking status (current/former/never), hypertension treatment (Y/N), statin (Y/N), aspirin therapy (Y/N).

Your current 10-year ASCVD risk will then be calculated. Be sure to review your risk estimate and the associated ACC treatment advice with your doctor. Please note the web version of this calculator is also available as a mobile app from the Mac App Store or from Google Play.
