Statement of Need

More than 40% of U.S. adults now live with obesity, and incretin-based therapies have transformed what treatment can achieve. Yet the same clinical experience that produced those gains has exposed their limits: response varies widely from patient to patient, persistence is often short, and weight is regained when effective therapy stops. Guidelines increasingly frame obesity as a chronic, relapsing disease — but many clinicians still approach it as a single problem with a single solution, and remain unfamiliar with the emerging biology, including amylin, a naturally occurring satiety hormone that regulates appetite through mechanisms distinct from GLP-1.

In this opening conversation, Lee M. Kaplan, MD, PhD, joins series chair Robert F. Kushner, MD, to make a deceptively simple case: because no single mechanism drives obesity in every patient, no single mechanism will treat it in every patient. Drawing on the logic clinicians already apply to hypertension and diabetes, Dr. Kaplan explains why a heterogeneous disease demands multiple, complementary treatment pathways — and why amylin represents a genuinely new “current” in that effort.

In the first activity of this CMEO Snack series, expert faculty will set the stage for the series: the biology of amylin, the promise of phenotype-guided care, and the emerging trial data that follow across three more expert conversations. Short, practical, and forward-looking — an essential primer for any clinician preparing for the next era of obesity care.
