Incorporating Collaborative Care Strategies to Enhance Linkage to Care and Treatment Initiation for Viral Hepatitis

Statement of Need

This activity focuses on one of the most persistent challenges in global hepatitis management: ensuring that patients who test positive for hepatitis B (HBV), hepatitis C (HCV), or hepatitis D (HDV) are successfully connected to ongoing care. Despite the availability of effective therapies, a substantial proportion of patients are lost to follow-up after diagnosis due to impeding social drivers of health, stigma associated with infectious disease, limited care coordination, and other structural barriers. In this interactive session, clinicians and researchers from multiple global regions share real-world strategies to support patient engagement, streamline care transitions, and build trust within affected communities. The discussion emphasizes practical solutions that health systems and individual providers can implement to reduce attrition along the care continuum and improve long-term outcomes for individuals and communities.

This CME Outfitters Snack activity is the second in the four-part series Linking Knowledge to Action: Empowering HCPs in Screening and Treatment of Patients Across the Viral Hepatitis Spectrum.
