Statement of Need
With more treatment options available for severe and persistent mental illness than ever before, clinicians can use pharmacologic differences between antipsychotic medications to target treatment to individual patients’ needs. However, expanded options have increased the complexity of clinical decisions. Clinicians need to match medication choices with short- and long-term goals, prioritize pharmacologic management of target symptoms, and minimize side effects and long-term medical complications. Data from clinical trials, while very important, are not sufficient to resolve all the treatment questions clinicians face. Because clinical trials are basically population studies with results reflecting averages from groups of patients, they provide limited guidance concerning individual patients.(1) Patients in clinical trials may also not be representative of those seen in real-world clinical practice, many of whom would not have met entrance criteria for clinical trials of drugs they are receiving.(2) Improved understanding of antipsychotic pharmacology (efficacy and safety data, pharmacodynamics, pharmacokinetics) can help psychiatrists apply clinical trial data to real-world clinical situations. Expert Knowledge Systems recently surveyed experts in antipsychotic pharmacology. Results of this survey along with empirical data from clinical trials and other sources were used to develop “Roadmap” recommendations to help clinicians achieve the best outcomes in treating patients with psychotic disorders. This CME Outfitters/psychCME TV activity presents highlights from the Roadmap recommendations and discusses how specialists in the treatment of severe mental illness can integrate and prioritize pharmacologic information to achieve treatment goals for individual patients.

Learning Objectives
At the end of this CE activity, participants should be able to:
- Contrast the pharmacologic profiles (efficacy, side effects, pharmacodynamics, pharmacokinetics) of the different antipsychotics.
- Identify the most appropriate antipsychotics based on characteristics of individual patients.
- Apply knowledge of pharmacology and patient characteristics to provide individualized treatment for psychosis in common clinical situations.
